Almost 20 years have passed since the publication of Felix Fleischner’s classic article on pleural fluid [1]. We review several important observations made in the interim and collate them with Fleischner’s basic tenets. The origins of some typical radiographic manifestations of pleural fluid are illustrated, including the meniscus configuration, subpulmonary effusion, apical fluid on supine radiographs, and interlobar effusion. Several radiographic appearances are explained with anatomic specimens. Our recent study of the anatomy of the interlobar fissures [2] facilitates the understanding of interlobar fluid collections.
