Cluster Headache: On The Inadequacy Of Existing Hypotheses Concerning The Origin Of The Autonomic Phenomena

The existence of a "central" mechanism underlying cluster headache has been advocated by several investigators (1-3): even though the pain maximum in cluster headache is in the ocular/periocular area, the substrate for the pain may be in a distant area, extracranially or intracranially. Various types of information concerning cluster headache fit with the idea of a central origin. In recent years, however, the search for pathogenetic models in cluster headache seems largely to have concentrated on peripheral structures, vessels, nerves, and ganglia.
